As a Caregiver II, you’ll provide personalized, tailored care based on each client’s unique needs and care plan. You will foster trusting relationships, ensuring clients’ dignity, comfort, and well-being. We seek dedicated, empathetic, and reliable professionals who are committed to making a meaningful impact and elevating the standard of in-home care through adhering to professional boundaries and fostering a collaborative environment.

• Provide personal caregiving services in accordance with an established plan of direct care.
• Provide for the personal needs and comfort of people in their own homes.
• Assist patients with activities of daily living.
• Perform general housekeeping activities.
• Provide transportation assistance to and from activities and appointments.
• Offer compassionate companionship to clients.
• Serves as the care plan liaison between the client served and the branch.
• Actively supports and represents the company's private pay initiatives including but not limited:  Taking an active part in growing referral sources when opportunities arise, working knowledge of marketing resources, recruitment, referrals, etc. 
• Assist company’s goal of being a primary agency of choice of homecare private pay clientele.
• Other duties as assigned.

• HS Diploma/GED Preferred required, additional professional certifications or associate degree is a plus
• 1+ years of accumulated experience in HomeCare/Long-Term Care setting. (Internal Candidates must have a minimum 6 months employed with AWC)
• Minimum of at least one letter of recommendation
• Dependable/Flexible/Available on evenings and weekends
• Bilingual preferred but not required
• Valid Driver’s License in good standing required from current state of residency
• Meet company standards for an approved driver
• Must pass company background check and applicable State required background checks
• Strong ability to communicate both verbally and written
• Capable of working responsibly with confidential information
• Must be able to work independently or collaborative environment
• Must demonstrate strong interpersonal and communication skills
• Accountable and goal-driven professional who takes pride in providing positive care and customer service
• Extended Continued Education and willingness to actively take part in ongoing education/training
• Ability to travel between clients as needed
• Must have Reliable Transportation/Auto Insurance
